1. Contact us by phone (615-234-3223) to book your appointment.
2. Print and complete the forms below.
3. Bring your completed forms to the office at Suite 101 of St. Thomas Medical Plaza West on the date booked.
   

Consultation Forms:
	Fact Sheet This form is used to collect basic personal and insurance information.
	Medical History Form This form is used to collect information about your medical history.
	Patient Agreement This document is the basic agreement between patient and doctor.
